Yes, all submissions are anonymous unless you choose to provide identifying information through the optional questions.

Please note that, because the Mascot Feedback form is collecting files for audio/video responses, it requires that respondents login to a Google account.

• • • This setting is required by Google for all Forms that accept file uploads. Disabling this setting is not an option without removing the ability to upload audio/video responses.

    • We value everyone's feedback and want to provide multiple ways of sharing this feedback, including audio/video recordings.

    • Even though Google is requiring you to login, your email address is not automatically being recorded by the form. Again, the only identifying information being recorded is the information you supply through the optional questions in the form (i.e. name, email address, phone number).

If you do not have a Google account, you can sign up for one using your existing email address by following the steps outlined in this help article.
